SPECIAL
HEALTH ASSISTANCE PROVISION (SHAP)
Beginning January 1, 2007, Acclaris will administer the
Special Health Assistance Provision (SHAP) program on behalf of IBM.
Previously, this program was administered by FBD Consulting, Inc.
If you or an eligible dependent is enrolled in
Medicare, you may be eligible for SHAP of the IBM Medical Plan and receive a
partial reimbursement of your MEDICARE "B" premiums. Eligibility details are
available on the SHAP claim form.
As part of this strategic outsourcing arrangement with Acclaris, IBM's Special
Health Assistance Programs (SHAP) -- for eligible retirees -- will be
administered by Acclaris. To submit a claim form , you must use the
new form and send it directly to Acclaris. Forms are available on the
following Acclaris Web site or by calling the IBM Employee Services Center.
